Sketchbook Error

I get an error mssg,

"All fields are mandatory; please fill them in."

- when I try to upload a new sketchbook item. I only see Create or Add to Existing Sketchbook, Title , & File, and filled them all in so I don't think it's user error.

Using Safari 3 at the moment.
